Parental leave

All documents for payment of benefits are accepted only with the provision of power of attorney.

Only the head of the organization has the right to submit documents for payment of benefits without a power of attorney.

If the head of the organization has delegated the right to sign to another employee, then a certified copy of the order defining this right is provided.

Temporary Disability Allowance

a) a statement from the insured person;

b) disability certificate.

In the event of temporary incapacity for work as a result of an industrial accident or occupational disease, an act of an industrial accident (in form H-1) or an act of an occupational disease (or copies of the investigation materials, if the investigation is continued) are additionally submitted. Prior to the submission of these documents, the benefit is assigned and paid in accordance with the provisions of the legislation on compulsory social insurance in case of temporary disability and in connection with maternity.

Pregnancy and childbirth allowance and one-time allowance for women who have been registered in the early stages of pregnancy

a) a statement from the insured person;

b) disability certificate.

c) for the appointment and payment of a lump-sum benefit to women who registered at the early stages of pregnancy, a certificate from the medical institution that registered the woman at the early stages of pregnancy is submitted.

One-time childbirth allowance

a) an application for the appointment of benefits;

b) a certificate of birth of a child issued by the civil registry office Form 24, Form 26 (original), or a copy of the child’s birth certificate if issued by a consular office of the Russian Federation, or a copy of another document confirming the birth of a child issued by a competent foreign authority, copies are certified by the employer. When contacting the territorial body of the FSS of the Russian Federation directly to the insured person in the event that the insurer terminates the activity if it is impossible to establish its actual location, originals and copies of documents are provided);

c) certificate from the place of work (service, social protection agency of the population at the place of residence) of the other parent that no allowance was granted, in case both parents work (serve), and also if one of the child’s parents does not work ( does not serve) or is studying full-time education in professional educational organizations, educational institutions of higher education, educational organizations of additional professional education and scientific organizations, and the other parent of the child works (serves) – origin al;

e) For persons replacing parents (guardian, adoptive parent, adoptive parent): copy of the decision to establish custody of the child (copy of the court’s decision on adoption adopted by law, copy of the agreement on transferring the child to foster care). The policyholder submits certified copies of the documents, the insured person, when applying to the territorial body of the FSS of the Russian Federation, presents the originals and copies of the documents;

e) certificate of divorce, if the marriage is terminated.

Monthly child care allowance

a) an application for the appointment of benefits;

b) a copy of the order for the provision of parental leave, certified by the insured;

c) a copy certified by the employer of the birth certificate (adoption document) of the child being cared for (When contacting the territorial body of the FSS of the Russian Federation directly insured person, if the insurer terminates the activity if it is impossible to establish its actual location, the original and copy are provided);

d) a copy of the birth certificate (adoption document, death certificate) of one of the previous children certified by the employer (When contacting the territorial body of the FSS of the Russian Federation directly insured person if the insurer terminates the activity if it is impossible to establish its actual location, the original and copy are provided). If this document is not submitted, if it is necessary to calculate the benefit in the minimum amount, the minimum benefit amount will be applied in the amount corresponding to the first child. Failure to submit the document specified in clause 4 g) is not a ground for refusing to grant benefits;

e) a copy of the decision to establish custody of the child, certified by the employer (if the insured person appeals, the original and a copy);

g) certificate from the place of work (school, service) of the mother (father, both parents) of the child that she (he, they) does not use the specified leave and does not receive benefits, and if the mother (father, both parents) child does not work (does not study, does not serve) – a certificate from the social protection authorities at the place of residence of the mother, father of the child about the non-receipt of the monthly child care allowance – for one of the parents in appropriate cases, as well as for those actually caring for child instead of mother (father, both parents) child:

– in case of continuation of the payment previously appointed and carried out by the employer, a copy certified by the employer shall be submitted;

– on newly assigned payments, the original is submitted;

– upon application of the insured person, the original and a copy shall be submitted.

Persons subject to compulsory social insurance in case of temporary disability and in connection with maternity employed by several policyholders, when applying to one of them of their choice to assign a monthly child care benefit, additionally submit a certificate (certificate) from the place of work (service, other activities) from another policyholder (other policyholders) that the appointment and payment of benefits by this policyholder is not carried out.

Payment of the additional leave of the insured person (in addition to the annual paid leave established by the legislation of the Russian Federation) for the entire period of treatment and travel to the place of treatment and back

c) the order of the insurer on the provision of leave to the insured person (a copy certified by the employer).

Reimbursement to the employer for the payment of social benefits for funeral

a) a statement of the insured;

b) death certificate issued by the registry office (original).

Reimbursement of the cost of a guaranteed list of services for the burial of a specialized service for the funeral business

Reimbursement of the insurer’s expenses for payment of 4 additional days off for the care of children with disabilities and disabled from childhood until they reach the age of 18 years
