Cyanocobalamin-Darnitsa as a metabolite activates the metabolism of carbohydrates (Carbohydrates – one of the main components of cells and tissues of living organisms. Provide all living cells with energy (glucose and its storage forms – starch, glycogen), participate in the body’s defense reactions (immunity). From foodstuffs vegetables, fruit, flour products are most rich with carbohydrates. Used as medicines (heparin, cardiac glycosides, some antibiotics). An elevated content of certain carbohydrates in the blood and urine is an important diagnostic sign of certain diseases (diabetes mellitus). The daily human need for carbohydrates is 400-450 g), proteins (Squirrels – natural macromolecular organic compounds. Proteins play an extremely important role: they are the basis of the process of vital activity, are involved in building cells and tissues, are biocatalysts (enzymes), hormones, respiratory pigments (hemoglobins), protective substances (immunoglobulins), etc.) and lipids (Lipids – An extensive group of natural organic compounds, including fats and fat-like substances. Contained in all living cells. They form the body’s energy reserve, participate in the transmission of nerve impulses, in the creation of water-repellent and insulating covers, etc.), participate in the synthesis of labile methyl groups, in the formation of choline (Choline (from Greek. choly – bile) – vitamin B. Contained in the seeds of cereal, legumes, beetroot and other vegetable products, yeast, liver. Choline is part of acetylcholine, which plays an important role in the exchange
Pharmacokinetics. In the human body, it is synthesized by intestinal microflora, from where it enters the organs, accumulating in the greatest amounts in the kidneys, liver, intestinal wall, turns into a coenzyme form – adenosylcobalamin or cobamamide, which is the active form of vitamin B12.
Indications for use
Cyanocobalamin-Darnitsa is prescribed for the treatment of malignant, post-hemorrhagic and iron deficiency anemias (Iron-deficiency anemia – anemia due to iron deficiency, belongs to the hypochromic anemia group, accounts for 80–95% of all forms of anemia), aplastic anemia (Aplastic anemia – anemia associated with the absence or lack of spinal cord function in children, anemia (Anemia – a group of diseases characterized by a decrease in the blood of red blood cells or hemoglobin) nutritional (Alimentary – associated with food, food) nature, as well as caused by toxic (Toxic – poisonous, harmful to the body) substances and drugs, other anemias that occur with a deficiency of vitamin B12, regardless of the causes of the deficiency (gastric resection, helminthic invasions (Invasion – the introduction into the human body of parasites and pathogens of infectious diseases), a violation of the process of absorption from the intestine, pregnancy and
Dosage and administration
Cyanocobalamin-Darnitsa is administered intramuscularly, subcutaneously, or intravenously, and with funicular myelosis and amyotrophic lateral sclerosis also intralyumbally. In case of B12-deficient anemia, the drug is used in doses of 100–200 mcg (0.1–0.2 mg) every other day until remission is achieved. When symptoms of the funicular myelosis appear and with macrocytic anemias with damage to the nervous system, cyanocobalamin-Darnitsa is used in a single dose of 400-500 μg (0.4-0.5 mg) and more. The first week is administered daily, and then with an interval between injections of 5-7 days (at the same time, folic acid is prescribed). In severe cases, injected into the spinal canal, starting with a single dose of 15-30 mg, and with each subsequent injection (Injection – injection, subcutaneous, intramuscular, intravenous and other administration of small amounts of solutions (mainly medicines) into the tissues (vessels) of the body, increasing the dose (50, 100, 150, 200 μg). Intra-lumbar injections are made every 3 days, for a total of 8-10 injections. During remission in the absence of funicular myelosis to maintain therapy (Therapy – 1. The field of medicine that studies internal diseases is one of the oldest and most important medical specialties. 2. A part of a word or phrase used to designate a type of treatment (oxygen therapy \; hemotherapy – treatment with blood products)) is prescribed 100 µg twice a month, in the presence of neurological symptoms, 200-400 µg 2-4 times per month. For posthemorrhagic and iron deficiency anemia, 30-100 mcg are prescribed 2-3 times a week. When aplastic anemia in children is administered at 100 mg before the onset of clinical and hematological improvement. With nutritional anemia in early childhood and anemia in premature babies, 30 μg is prescribed for 15 days. With amyotrophic lateral sclerosis (Amyotrophic lateral sclerosis – organic disease of the central nervous system, which consists of steadily progressive spastic-atrophic paresis of the extremities and the appearance of bulbar disorders), encephalomyelitis, neurological with pain syndrome (Pain syndrome – subjective sensation that appears due to the impact on the body of superstrong or destructive stimuli. There are pain syndromes of the head, face, mouth, back, etc.) diseases are administered in increasing doses from 200 to 500 micrograms per injection (with an improvement of 100 micrograms per day). The course of treatment up to 2 weeks. For injuries of the peripheral nerves, 200-400 μg are prescribed 1 time per 2 days for 40-45 days. Young children with dystrophic conditions after illness, Down’s disease, cerebral palsy (Paralysis – loss of strength in voluntary muscle movements after damage or pathological changes of the nerve that innervates these muscles) is prescribed by 15-30 μg every other day. For hepatitis and cirrhosis of the liver, adults and children are prescribed 30–60 mcg per day or 100 mcg every other day for 25–40 days. With diabetic neuropathies (Neuropathy – features of the development of the nervous system with a child’s tendency to neurotic reactions. In the foreign literature, neuropathy refers to a disease of the peripheral nerves of ischemic and toxic origin (for example, in diabetes), sprue, radiation sickness is administered in 60-100 mcg daily for 20-30 days. The duration of treatment with Cyanocobalamin-Darnitsa and repeated courses depend on the nature of the disease and the effectiveness of the treatment.
In case of hypersensitivity to Cyanocobalamin-Darnitsa, allergic phenomena, headache, dizziness, nervous agitation, pain in the region of the heart, tachycardia (Tachycardia – increase in heart rate to 100 or more beats per minute. It occurs with physical and nervous tension, diseases of the cardiovascular and nervous systems, diseases of the endocrine glands, etc.). In these cases, the use of cyanocobalamin-Darnitsa is temporarily or completely abolished. If necessary, treatment with Cyanocobalamin-Darnitsa is resumed, starting with small doses of the drug (15-30 μg).
The use of Cyanocobalamin-Darnitsa is contraindicated in erythremia, erythrocytosis, neoplasms with the exception of cases involving megaloblastic anemia and deficiency of vitamin B12 in acute thromboembolic diseases.
Interaction with other drugs
Co-administration of vitamins B12, B1 and B6 in one syringe is not recommended, since the cobalt ion contributes to the destruction of other vitamins. It should also be borne in mind that vitamin B12 can increase allergic reactions caused by vitamin B1.
When treating Cyanocobalamin-Darnitsa, it is necessary to systematically perform a blood test. In case of a development of leuko- and erythrocytosis, the dose of the drug should be reduced or temporarily interrupted. It is also necessary to control blood clotting, being careful with the tendency to thrombosis. Caution is also prescribed to patients with angina.Angina pectoris – syndrome caused by myocardial ischemia and characterized by an episodic appearance of discomfort or pressure in the precardiac region, which in typical cases occur during exercise and disappear after it stops or taking nitroglycerin under the tongue (exertional angina)), and in smaller doses.
