On August 30, the 17-year-old Walker Smith underwent a sex change operation and officially became Amy Smith. The transgender allowed National Geographic photographer Lynn Johnson to be present in the operating room.

Amy decided to cover all the details of an intimate operation. how the glans head becomes the clitoris, how the skin of the penis becomes the labia minora. The skin of the scrotum is also used to form the vagina.

Amy’s mother and her twin brother Caleb, of course, were shocked by her decision, but generally supported.

Surgical sex change

Surgical gender reassignment is the only real possibility of treating gender dysphoria (transsexuality) – the discrepancy between the gender identity of the individual and his anatomical gender. Transsexual surgery allows you to achieve an effect that is not possible when taking hormonal drugs (possibly a change in both secondary and primary sexual characteristics).

Contraindications

This operation has a number of contraindications:

  • mental illness;
  • young (up to 18) and old age;
  • homosexuality;
  • alcoholism;
  • severe illness of any of the systems of the body.

How is the sex change

A patient who wondered How to change the gender? should be ready for long-term preoperative preparation, including several stages. So, for the diagnosis of transsexualism, a special commission is held, which includes at least three psychiatrists. Also, the social adaptation (living for a year in the role of the opposite sex), observation by a number of specialists and hormonal training are mandatory.

Permission for transsexual operations is issued subject to the following conditions:

  • a patient who has realized his transsexuality at an early age constantly feels sexually inadequate;
  • the probability of committing suicide due to the discrepancy between real sex and inner self-awareness;
  • an adequate level of social adaptation to new living conditions.

This stage can last up to two years.

Methods of surgical change

Transgender operations involve the transformation of the male into the female and vice versa. In the first case, surgeons remove the tektikulu and the body of the penis, and then create the vagina. This intervention can be complemented by feminizing mamoplasty (augmentation and repositioning of the breast), liposuction at the waist, feminizing facial surgery (changing the cheekbones, eyebrows, nose, chin, eyelids).

With the transformation of the female to the male, all primary sexual characteristics are removed and the penis is created. If necessary, an aesthetic correction is carried out: liposuction of excess adipose tissue, changing the shape of the chin and calf muscles.

The duration of this intervention is from 4 to 5 hours. The main stages of the operation:

  • removal of the testicles (orchiectomy);
  • vaginoplasty (the formation of a new vagina, labia and clitoris using tissues of the penis, scrotum or sigmoid colon);
  • shortening the urethra;
  • removal of the cavernous bodies;
  • plastic pubis.

How to do surgery to change the female to the male

This surgical procedure includes the following steps:

  • mastectomy with preservation of the nipple zone and skin patches;
  • removal of the uterus (hysterectomy), ovaries (ovarectomy) and appendages (adnexectomy);
  • colpectomy (removal of the vagina). At the same time, fragments of the vulva intended to form the scrotum and penoid (artificial penis) are preserved;
  • phalloplasty (formation of peniodes). The place of formation of the new organ becomes the forearm of the nondominant hand, into which the artificial urethra is implanted.

.

The head of the member is created by plastic method. After lengthening the natural urethra, a penoid formed on the forearm with soft tissues, vessels and nerve fibers is transferred to it. An important advantage of this operation is the preservation of the clitoris, which is placed on the foam.

The patient spends the first few days after the operation in the intensive care unit, after which he moves to a regular ward. On the sixth day, doctors are allowed to stand up for the first time. On the twelfth day, a catheter is removed from the bladder, after which it becomes possible to urinate while standing. Discharge of the patient occurs in two weeks.

Full sex life (male type) requires the subsequent implantation of an erectile prosthesis – a hydraulic system with a pump that is placed in the neo-scrotum. The reservoir, which injects an erection fluid, is located under the peritoneal muscles. Thanks to the pump, sterile saline is applied to the special silicone rods inside the penoid, causing an erection. After the end of intercourse, the fluid returns to the reservoir. In parallel with these manipulations, silicone testicular prostheses are placed in an artificial scrotum.

Implantation of an erection prosthesis is performed 8–9 months after the main operation.

Transgender Hormone Therapy

Treatment of transsexualism with the help of drugs is carried out in two stages: before and after surgery. In the preoperative period, patients are prescribed hormone replacement therapy, which transforms the appearance in accordance with the desired sex. Biological women are prescribed injections of male hormones (androgens), men – taking estrogen (in the form of tablets, patches, gels) and antiandrogenic drugs (if necessary).

As a result of hormonal therapy significant external changes occur:

  • in women, the timbre of the voice changes, the facial features grow coarser, the growth of hair on the body and face is activated. In some cases, an increase in leg size and growth of the hands are observed;
  • in men, the volume of hair on the body and face decreases, the mammary glands increase, the voice changes.

After the gender reassignment surgery has been successfully performed, patients are given lifelong hormone replacement therapy.

Sex after sex change

Sex change does not preclude an orgasm: the genitals can take part in sexual intercourse, and the ability to integrate penis tissue into new organs allows you to retain the ability to receive an orgasm. Many correction techniques retain erogenous patches on the genitals.

The final cost of the operation is determined only on internal consultation based on the complexity of the surgical treatment and the patient’s particularities and can be changed, both upwards and downwards.

How much does a sex change operation cost?

A bit of history about sex reassignment operations. For the first time, a transsexual intervention was conducted in Denmark in 1931. But until the mid-60s, these operations remained unique. Everything has changed since the second half of the last century. The first specialized clinics where transsexuals helped were established in France. In 1978, an international association of surgeons specializing in gender reassignment surgery was created in the United States. In the USSR for the first time such intervention was carried out before 1991.

Third floor

In our time, sex change surgery is quite common. For example, in Britain between 2000 and 2010, 853 men lay under the surgeon’s knife to become women. According to statistics, the average age of people going to change sex – 29 years.

In the United States, between 100 and 500 sex change operations are performed annually. However, the leader in the number of surgical interventions remains Thailand. Here you can find a record number of young ladies who were once guys, and vice versa. In Thailand, transsexuals have been allocated a separate gender – the third. According to Buddhist beliefs, transsexuals are unhappy souls punished for past life acts in such an extravagant way. The cost of a sex change operation in Thailand is one of the lowest in the world, with a very high level of local medical care. Officially, about ten thousand transgender people live in the country, but in reality there are several times more of them.

The Islamic Republic of Iran, as is known, lives under Sharia law. Nevertheless, there have been sex reassignment operations since 1979. Basically, men with homosexual inclinations are solved for the operation. Cohabitation with a man of the same sex in Iran is punishable by death, so gay people agree to turn into women.

In Iran, a sex change operation costs about five thousand dollars, but if the patient does not have this amount, the state pays up to 50% of the cost. After the operation, the newly-minted woman is given a new birth certificate, and since transsexuals have problems with employment, the state provides them with a loan to develop their own business.

As a result of this policy, Iranian surgeons do more gender reassignment operations than anywhere else in the world (with the exception of Thailand). The number of transsexuals in Iran is about twenty thousand.

We had the first sex change operation in the days of the USSR. In Ukraine, the cost of a sex change operation is officially free for citizens # 8212; in public health facilities. In commercial clinics it will cost from 2 thousand dollars. But despite the low price, many patients prefer to do a sex change not in Ukraine, but in Russia or Thailand.

Types of surgical body changes

  1. Breast correction. Removed breast tissue, excess skin; nipples move.
  2. Reductive mammoplasty. Eliminate some of the breast tissue so that the chest resembles the male.
  3. Vaginectomy. Remove the vagina.
  4. Colpokleisis. The vagina is closed surgically.
  5. Metodioplasty. The clitoris is lengthened to form a small penis.
  6. Phalloplasty. The penis is created using tissue from other parts of the body.
  7. Ovariectomy. Ovaries and fallopian tubes are removed.
  8. Hysterectomy. The uterus is removed
  9. Salpingovaroectomy. The uterus is removed along with the appendages.
  10. Scrotoplasty. A scrotum and testicles are created.

Also possible are operations such as removing subcutaneous fat (liposuction) and placing implants in the calves, chest, and chin so that these parts of the body look masculine.

Types of floor correction operations

Surgical correction of sex can be of two types:

  • sex change operation from male to female – MtF (Male to Female),
  • The operation to change sex from female to male – FtM (Female to Male).

The second type is much less common, as more complex surgical interventions are required. In addition, the correction of the second type of sex (FtM) is a more expensive procedure.

How do women change their sex?

The operation to correct the sex, if it is necessary to become a man, goes through several stages:

  • remove mammary glands, change the shape of the nipple;
  • get rid of the uterus, ovaries, fallopian tubes;
  • suture or remove the vagina;

Some time after the above manipulations, the penis is reconstructed. To make the appearance more consistent with gender, some patients need liposuction, face surgery, and calves.

From boy to girl

During a sex change operation, a man is removed from the penis and forms the vagina in the genital area. It is created from the tissues of the penis and scrotum, or skin grafts from other parts of the body and intestinal fragment. Before surgery to remove the testicles, long-term hormone therapy is required to reduce the level of testosterone and its derivatives. After removal of the testicles, the production of male hormone is stopped, which makes it possible to reduce the number of drugs taken. After surgery, the patient accepts female sex hormones (estrogens) to change the contours of the body and increase the mammary glands.

Indications for surgical change of male to female

The indication for surgical sex change is transsexuality. Operations must be preceded by a long period of psychological adaptation to a new gender role.

There are a number of methods to determine whether transsexuality is a real problem in a particular patient. The psychological conclusion must confirm that the patient is in an adequate mental state and does not suffer from various mental illnesses.

Contraindications for surgical change of male to female

Contraindications to surgical sex change include:

  • the lack of a confirmed transgender diagnosis confirmed by qualified specialists;
  • alcoholism;
  • homosexuality;
  • the presence of severe systemic diseases, including mental ones;
  • age up to 18 years;
  • elderly age.

Cost of sex change surgery in Russia

As in the rest of the world, in Russia it is impossible to simply take and change the sex by coming to a plastic surgeon. To change the sex, the patient must have a certificate with a diagnosis of transsexualism, which is issued by the mental dispensary after two years of observation of the patient. In addition, the patient must be over 21 years old and have no contraindications. By the way, on the territory of the Russian Federation sex change operations are not done to homosexuals.

Gender reassignment is not one complete operation. This is a whole range of effects – hormone therapy, breast reduction or enlargement, facial correction and, of course, phalloplasty (the formation of the penis to go from woman to man) or the formation of the vagina (to go from man to woman). Some steps, including the last one, can be skipped for one reason or another, but after a series of interventions, the person usually still changes documents, issuing a passport with a different gender and name.

The cost of consulting a psychiatrist in Russia is on average about 15 thousand rubles. The operation itself in domestic clinics will cost 550 thousand rubles. However, few people decide to go for such a serious operation in Russia, foreign surgeons seem to be more reliable.
